If you have any questions about my policies or any information contained in these forms, I will be happy to address them in our first session together.
If you do not have access to a printer, please let me know so that I can leave these forms for you in the waiting room. Please arrive at least 20-30 minutes early to give yourself adequate time to fill out these forms.
